How to write a hockey player resume/bio?

I am going to a Free Agent camp later in the summer and I need to write a resume/bio but I have no idea how to come up with one.
Does anyone (especially LITY) know how to make one?
It says that specifically on the sheet. Here is a quote:
"Send the attached registration form and your player resume/bio to:"

A bio is different from a resume; the latter talks about your skills, job experience and education. The Bio talks about what you want to talk about: in your case, only Hockey and nothing else. Write how long you’ve played, when, where, what positions, your coaches, your tournaments, etc. If you have a pic of yourself put that on the bio; and what objective you have for yourself in Hockey….. for starters.
i get in fights and i have some teeth plus i skate der
